Please be informed of the following important enrollment dates:

• July 1, 2024 (Monday): Kinder to Grade 3

• July 2, 2024 (Tuesday): Grades 4 to 6

• July 3, 2024 (Wednesday): Grades 7 to 8

• July 4, 2024 (Thursday): Grades 9 to 10

• July 5, 2024 (Friday): Grades 11 to 12

Requirements needed:

For Old Students:

• Form 138 (Original Report Card)

For New Students:

• Baptismal Certificate

• PSA (Photocopy)

• Form 137

• Form 138

Visit us at St. Augustine School - Mendez, Inc from 8AM to 4PM. Enrollment area is located in front of Registrar, Cashier and Guidance Office.

To ensure a smooth and organized enrollment process, please make sure to follow the schedule designated for your grade level and have your requirements ready.
